It shows how Sayo created her new identity as Ikuko to help battler as he recovered from brain damage nearly drowning trying to save her, and how her guilt from her selfish suicide attempt fueled her drive to help Battler recover, as he was the one person who could forgive her and tell her to live on despite her sins.
As well as her Gaslighting Battler into believing it was a car accident that caused his injury so he wouldn't stress about Rokkenjima until he was ready, and other specifics of the process of fabricating his identity and the connecting of the numerous hints left through episodes 3-7 about Ikuko's identity.
The 'Tantrum' that Battler had that left him in a wheelchair was when he realized Ikuko's identity and what she had done for years, but he eventually comes to terms with it and agrees with her motivations.
