This varies so much depending on the game. Some games like Chrono Trigger did it really well, and even though Crono never says a word he still has an obvious personality, kinda like Wind Waker Link. Then there's stuff like Xenoblade X where your character is a mindless doll who stands around like a statue during cut-scenes. One step even worse is Dragon Quest IX where the entire party consists of mindless dolls.

To be fair with Mass Effect it's implied the Council knew about them but didn't want to cause mass panics across the galaxy that an ancient race of all-destroying tomb warships that can irreversibly brainwash people and have successfully culled the galaxy of all intelligent life for millions of years every 50k years is coming to kill everyone

and also that, even if they knew about them and accepted the threat, they were probably dealing with a million more immediate crises every day, so it was at the bottom of their list even though it was serious

I like it more when good choice impacts story and is helpful long-term while bad choice impacts gameplay and is impactful short-term.
No clue what neutral choice could even do differently though but maybe that's because we need to move from the alignment choice style and have something like preference choice style

I think the reason a lot of MMOs do this is because of the release style of expansions. They want the player base to get hyped so each expansion they keep upping the scale of things more and more, but then you get to the point of WoW where you've stopped the end of the world a 1000 times and be a literal god among men, everyone should known your name, but instead some random dude asking you fetch berries for him is insulting you and calling you a worthless piece of shit.

Though this trope has been dying out, a lot of modern RPGs I've seen nowadays do make an effort to actually make status effects/debuffs and the like useful.
