My mind is a bit hazy right now but I miss them like you wouldn't believe.
Ace Combat 7, final mission. There is a mostly scripted event where one of the (somewhat) more significant voiced enemy-turned-ally pilots you face gets shot down follow a stage change, it's "supposed" to happen. But it's actually possible to save him, my friend did it by being fucking great with the EML. I've never managed it though, takes not just skill but a fair amount of luck since you need to come out of the cut and find it like immediately.
All those NPCs in Divinity 2 where you're trying to weaken Adramalihk.

You would not believe how much I savescummed to keep this guy alive as a kid. I think in singleplayer games any friendly death hits you a lot more. Especially in games where everything you see wants to kill you. I got stuck in that game as a kid and never bothered to finish it. I remember being pissed you couldn't save him.
Always and forever I wanted to save her as a kid so fucking bad I went back to the fucking mountain and ground to like 90 or some stupid shit when you can't possibly still have any fun so I could try to beat lavos at that stage I was a dumbfuck new to JRPGs and didn't know ng+ was a thing. I was so disappointed to find Dev End (which was a dupe anyway) rather then some secret end where zeal lived on. My only real lasting grudge with an otherwise perfect game.

From Max Payne 2? They don't die in a cutscene and it's not hard to escort them alive through the level. The whore never makes good on her offer "You do your job and (giggle) I'll do mine"

When I took my machine Connor playthrough too far and Hank killed himself.
Gameplay deaths > cutscene deaths It's cheap if done in a cutscene but when it's gameplay you know that your actions fucked everything up. I found Max Payne 2 creepy af too so having some people watching my back was a godsend. Max is MGTOW anyway.
Fallout 1, Dogmeat, died via forcefield in the Mariposa military base.

That samurai in Sekiro who helps you fight that fat drunkard boss at the Hirata Estate. I cleared all the enemies except the boss, but I wasn't aggressive enough during the boss fight and he died just before the boss did. RIP, friend. I'll do better in NG+
