Standards have changed over the years. Ever see the scene in Universal's Frankenstein movie where the monster throws the little girl into a lake? People were shocked and that scene and it was extremely controversial at the time. Nowadays people wouldn't bat an eyelash at that. The same thing has happened with video games people have become accepting of higher levels of violence. You also have to remember that similar to Frankenstein Doom predated a widespread rating system.

Nowadays with widespread use of the ESRB blame is usually just deflected back to parents because they shouldn't be letting their kids play games M rated games to begin with.

The Ultimate DOOM is just the final version of DOOM. It's the same game but with a fourth episode. I don't even know why they have both up at the same time.

Actually scratch that they don't have both. The DOOM you have is a completely different game made in 2016. They're both fun.
