The answer to almost all of your questions is to think really hard about the plot twist of DR1, look at the initial game covers for DR1, SDR2 and V3 and figure out what's weird about that.
The real Junko is missing from DR1's cover, Izuru is also missing and the Kokichi you saw in the game isn't the real deal. It's simple logic, Kokichi can't know so much unless he's actually related to the mastermind.
