Heartman is some dude who dies every time to find his family; and hasn't found them for 9 years in the afterlife. Cliff is a dude in the real world interfering with your simulation, possibly with knowledge of Mads. Mads is a person in the real world who wants you to live and grow; and makes himself available to challenge you in the simulation.

If there's a new game anyway it's probably already heavy in development, they recruited back Kazunobu Uehara (main programmer of mgs1-2-3,he left just after mgs4) at the company in early 2017 and he wasn't on any projects since. If he isn't in Contra rogue corps he has to work on MG with the rest of the MG team.
I'm sure the team can do a great MG even without Kojima.

The PS4 is pretty secure honestly, the only real stable hacks were userland stuff and they required months old firmware to boot.
