Natural Doctrine did this and made a plot point out of it with Pluton, doesn't really work, ends up being one of the most tedious kind of resource management, especially since in that game Pluton was a finite resource so your mages had to really avoid doing anything at all until it was time to drop the pluton nuke to turn to tables.
Doesn't help that the game was also X-Com taken to its logical extreme, which made the game neither a RPG nor a strategy game, but a trial and error puzzle with progressively less room for errors depending on the difficulty level, a single wrong move at any point of your progression in a stage and it was game over.

but my point still stands, I've never seen it done for anything in the "sentient D&D" realm; adventure, action, RPG, etc. The closest have been military-type games about playing with military toys, never anything fantasy or adventure-themed.
Also, most of the games that have done this have kind of hand-waved the setting into the background to be more environmental; you are playing as little plastic characters, and some of the levels are like a way-scaled-up version of a kitchen counter with cans on it, or whatever. But you don't really have the meta-narrative of the actual humans playing the game going on behind it, and I think that's what makes the setting interesting. At least, it was what made it interesting for the Fable 3 quest; it's funny because the world changes and responds to these omniscient-view narrator characters interacting off-screen and talking about what you're doing.

Build civiliation up on an individual character-based level with people who will live for hundred of years in the prime of their lives. Convincing someone that using some in-between medium is more efficient (instead of direct bartering) is the birth of currency, etc.
End-game consists of preparing for a randomized gauntlet of major disasters (barbarians, drought, famine,disease, animal attacks) to test whether the combination of character you made, civilization you built, and people you work with are going to live on to start civilization as a whole. Bad decisions in one area can be compensated for with good decisions in other areas and skillful play.

A comical tone would fit very nicely, as you go around trying to function as normally as you can while trying to keep a low profile. Also, would work in pretty much any setting.

the whole point is that judgement day suddenly happens, and the sun scorches the skin of any and all sinners. Thus most children are exempt from this curse while most adults nearly self immolate. There would be straight up demon enemies for the end game and you'd discover angelic weapons and armor as loot. The final boss would be god, just like my jrpg's.

Now that you've woken up the god stoped takeing calls, there's a skism in this faith thats just in the uncanny Valley between your beliefs and forgin gods, people are questioning if the evil was actually killed in the first place and some are proclaiming you a new evil awoken.
