If its in games like Zelda that give you a naming option but the main character has an established name, I just leave it be.
Otherwise I pick the name based on the character. Be it appearance or build or a combination of both.
For example I'm playing Bloodlines right now and felt that the name "Elizabeth" fit pretty well for a emo / edgy looking blood wizard geek.
