It really was fucking brilliant. The juxtaposition of the bright, vivid imagery mixed with the disturbing things that happen in the film really boggle the mind. Wish it had performances akin to Toni Collette from Hereditary, but overall I think it was a better film.

Because arthouse usually pushes the medium to it's fullest. An "arthouse" game typically only focuses on narrative or presentation rather than the whole "game" part of "video game."
I'd consider Silent Hill to be good arthouse, same with a lot of Suda51 games.

And to elaborate I think that what is considered "artsy" music is usually way different in tone than arthouse film. Each medium has a different set of candidates for artistic vision. For music it's both in parts musical theory, instrumentation talent, tone, lyricism, and more I'm probably missing. With film there's talent in framing, scripting, direction, emotional output and range from actors, set design, yadda yadda yadda.
For games it's primarily a composition of computer science, creation of game mechanics, level design, the psychological connection the player has with the primary avatar, the feelings emitted if you win or lose.
I'm kind of dumb but I feel that judging games on the same scale as movies or music is like judging music to a painting. They are entirely different concepts with different ideas intertwined.

Pathologic 1 and 2 are amazing works of art that work ONLY in the medium they're made, making the most out of the videogame format. Thats the thing about the ending that really is messed up, she appears happy but really shes on a cocktail of different drugs and emotional reactions inspired by those drugs and the strange circumstances she's experiencing. There's no way she's thinking with a clear head of any sort.
