It's kind of a moot point when most likely story NPCs can be killed along questlines just not at random intervals.
You could rack up a pretty high death count in The Witcher games for important sotry NPCs at critical junctures.

Yup, they can be very buggy. I just installed around 25 mods while I play Skyrim VR. I do enjoy the ability to have mods though. The Nexus is such a thriving community and 2077 lacking this is what disappoints me the most.
