They shouldn't, there was never been a good difficulty selection, either only one of them is good, which negates the point of having a difficulty select in the first place, or none of them are good, since the developers try to balance all of them and fail (or they don't try to balance them at all)

While I don't expect many people here to share my opinion, I do prefer not having difficulty settings for that exact reason.

Uhh... do you know what the definition of the term "Normal" is? If a game has a normal difficulty that is the creator's vision. If not, the default difficulty that is selected is the intended difficulty, like in Doom. Everything around those difficulties is artificial difficulty or pic related

You can have multiple visions, so sure. Just don't lock a difficulty behind completing the game unless it's a continued learning curve from the hardest normal mode. Timespinner and Bloodstained are retardedly easy on normal, and you need cheats to unlock other modes (which also have shit balance but it's better at least). And don't confuse the player, make sure they know what the intended difficulty level is.
