Now to answer your question. It depends. If the model's head looks exactly like character does in game, it's what we call a head hack. A head hack is basically stitching the head of one model, usually ripped from a game to another one, usually a DAZ body.
The second are fully custom models, they typically do not match the art style and are custom made like my own here.
