The player's end goal should be clear right from the start, while other elements are close enough to be points of interest, they do not interrupt, or force you to stop your direct objective.

The way I see it, in Narancia's case, Epitaph presents Diavolo with the momentary future [Narancia got fenced by King Crimson] since he's nearby.
King Crimson skips the 10 seconds of time that would include [The gang's interactions with each other / King Crimson takes Narancia and fences him].
Leaving the real world with normal interactions --> F E N C E D

Diabolo becomes ethereal (can't attack, can't be attacked) in Skipped time, consequently anything touching him is ethereal too.

In Trish's case

He used Epitaph when he heard them coming and saw Trish walk out of the opening door while Buccellati stood still, holding her hand. He created a makeshift guillotine and when the elevator approached the second floor from the top, he stopped it and skipped time. Trish walked out of the door, the guillotine severed her arm, and Buccellati pulled her hand back in. The doors shut and the elevator continued. When time resumed, Trish fainted and Diavolo caught her. He forced the elevator door open and climbed down the shaft, as Buccellati realized he was holding a severed hand.

King Crimson, on crack from Chariot Requiem, manifested behind everyone, grabbed Narancia (who was in the back and climbing the columns), and quickly chucked him into the fence. As soon as he propelled him, before Narancia could scream, he skipped time.

If he could attack during skipped time, he would have when we were watching, and he would've never gotten face-to-face with anyone. In fact, he could've just taken the arrow and stabbed himself, and no one would've been the wiser.

For Diavolo to use Epitaph and King Crimson before Dio uses The World, you'd have to be acknowledging the characters' personalities, because otherwise Dio would stop time first. However, acknowledging character personalities, Diavolo would punch Dio through the torso, which wouldn't kill him. Dio being a vampire, he'd survive and Diavolo would be screwed.

Except he doesn't. He only uses the skipped time to evade and dodge his opponents, and only after the time has skipped does Diavolo go for the kill. If Diavolo attacked within the skipped time, there would be no reason why he would wait until after he skips time to kill Bruno and Risotto.

Because despite being the villain, Diavolo is pretty level-headed and fair. He doesn't kill or harm innocents, and even his enemies he treats fairly. He gives them a quick, dignified death and doesn't go out of his way to do something like rape them.
