This is probably the smallest thing to get irritated by, but why do so many developers make this mistake? This is a road pattern made by cars and only cars. In the olden days the middle of the road would have been well trampled by horses pulling carts.

It is very trivial, but that also means its a very simple thing to fix. Just take the grass out. Bam, your road looks cleaner and is not glaringly inaccurate.

It is unlikely for a main tract between large cities, where merchants, armies, peasants, knights, courts and whatnot travel regularly, to look like that. Grass in the middle is a mechanic to show roads diverging, in this case it is better to be less historically accurate for the sake of gameplay.
rdr 2 absolutely got it right though. The roads with grass in the middle usully has high enough traffic in both directions so the grass is just the natural divider.
