Old games never saved the characters you'd take the time to make so I never bothered putting any effort into it.
So I default to using premades or if I'm lucky and the character creator isn't complete garbage, the randomize option. Then just a few simple edits from there.

Typically effeminate in physical features.
Has tomboyish traits, w/o going overboard.
Her physical appearance doesnt fully come off as tomboyish (like short haircuts or muscles. Typical long haircuts, or pony tails to keep hair length)
And if possible, less insecure mannerisms, and more "capable" mannerisms. (walk/idle animations)

I usually make my guy look as close to a placeholder player model as possible, by which I mean solid white skin, no hair, as few facial features as possible, least detailed clothing if necessary, etc.
The only acception was Drawn to Life where I tried to recreate the mannequin you're made from.
