When Lovecraft felt the need to describe colors it was usually something of note. In Bloodborne that translates to stuff like the yellow clock face hanging over the nightmare or the red and purple sky after you kill Rom. You know, things that contrast against the otherwise dark aesthetics the rest of the city holds. It's supposed to be grim dark, and colors are splashed in when something needs to be shown as different.

Comparing the cracks on the backs, I can attest to this in fact not being the smith.
So it's actually a different character, and not the smith just being effeminate as fuck from the front.

The movie only adapt the first arc, the english translation for the manga stopped at around volume 10 (there's more english volumes out there but that's because they divided each japanese volumes into severals smaller ones). I think the italian translation is still keeping up with the japanese release.
