You have to beat 100-Mario Challenge so many times to unlock most mystery mushroom costumes, then there are costumes exclusive to Super Expert mode and special courses. Plus you can unlock the skinny mushroom after beating all of the 10-Mario Challenge courses and I believe beating the fly swatter minigame gets you Builder Mario. Then you have to unlock all the medals for getting upvotes on your own courses.
