It was in the right place at the right time. It had a small following through the 90s. What made it explode in popularity was not what the game itself did, but what happened in the doujin comic scene at the time.
A bunch of companies got lawsuit happy and started targeting doujin studios because they didn't like them making smut out of their IPs and profiting off it. Fearing retribution from big studios, they looked to their fellow doujin creators for content. ZUN gave them their blessing and the rest is history.
The lawsuits eventually stopped and everything went back to business as usual, but the popularity stayed.
