In the trailer there are several points where both of his arms have very snappy movements, its very clear that you the player has direct control
over both of his arms. This allows you to maintain you balance when carrying heavy loads, and towards the end of the e3 2018 trailer you can see
that it allows you to place either one or both of your hands over your mouth to control your breathing. I genuinely wouldn't mind if one of death strandings
core gameplay loops is essentially a hardcore hiking/climbing and delivery simulator. Its also worth noting that kojima himself has said that you'll also be
able to play it as a more conventional shooter(hence the gun you saw in that e3 2018 trailer), but like metal gear solid running and gunning probably won't be
an ideal way to play.

There are currently only delivery missions
They all are randomly generated
No story, but they give you some insight on the "lore" of the world
after completing set numbers of missions you will see a random cutscene, on of the dozens
They are planning to add more content before the fall. Major update early next year. Full release by the end of 2022 fiscal year
