A Phoenix Down/Life spell can cause the an instant kill. The word "Death" appears.

They don't. Phoenix Down isn't a resurrection.

zero hp just means too injured to fight. probably the only FF where someone dies for real in a battle is in FF4, where Tella goes way into the negative before going down for good. Typical pen and paper RPG rules.

Don't remember the exact rules in 7 (equivalent to the Death spell with low probability?), but at the very least this doesn't normally happen. X potion is the only guaranteed kill.
