It's a separate game on a heavily modified engine. About a third of cool (G)ZDoom features come from supporting Strife (the other two thirds being Hexen).
It's alright, although you can tell Romero has lost his touch when it comes to God-tier level design. Not to say it is horrible, but it is lower quality than most of his other shit.

Already finished it, hated it. There's no way to progress through the levels by adapting to challenges on the fly, as should be the case in a fast paced action game like Doom. You instead have to scope out what comes next, count your ammo, and then formulate a cost-benefit analysis to decide which things you're going to bother to kill and the route you're going to take to avoid everything else. It's like it's designed specifically with speedrunning in mind.
I can definitely see how it will have a tremendous amount of appeal to people who are into that sort of shit though. All power to them.

If I wanted to waste 30 minutes per level killing bullet sponge demons with shotguns, I would just replay the first Hell Revealed. They at least give you SSG and properly designed levels.
Why are we still pretending that Doom is some pinnacle of level design?
It was like the third game in the world to have level design and it was better than the previous two, that's about its claim to fame.

E1 was actually well designed even to this day. E2 is debatable, E3 is eh and E4 is good except for the first two.
