Do I have to play 1 or can I start with 2?

You can start with 2 but it is only the Haruspex character route right now. The recommended first route is Bachelor because people explain things to you. Bachelor is new to town and people are quick to explain their strange ways and town while Haruspex is a native and all npcs talk to him/player as if he already knows everything about the town.
I recommend just the Bachelor story path from 1 then try 2 for your next path to appreciate the improvements and better understanding. Then either wait for Changeling or Changeling in 1 depending on how you feel after.
Although if you want to dive right into the upgraded remake that is 2 slipping the player path with the introductory lore and explanations go for it.

It's really hard to summarize it into some selling points because it interweaves so many themes in such ways that anyone who plays it would take away something different from it. The only thing I can promise you is that it's a legitimate work of art unlike anything else in gaming except for the devs' other game, The Void.
You have to take the plunge and see what it offers for yourself. If you can stick to the intentionally harrowing gameplay and possess some level of introspection, you'll come out of it with a new perspective on some things. Other user already summed it up, my main consideration would be that 1 is much rougher around the edges gameplay wise. 1's Bachelor is THE introduction you want to the game but I can already recommend going into 2 for the Haruspex, it's much more fleshed out. They had problems putting on meat for the later two characters in 1.

Is running necessary? I'd rather walk everywhere but I don't wanna be strapped for time by the end of the day.

The sound design is pretty good so headphones are a must. It's long but not as the first one since it's only one of the 3 routes you could take

Immersion is good, I even light a candle and incense while I play. It's more daylight tension than spooky horror but being in the dark still helps focus on just the experience of the Town and the Outbreak.
For length, iirc 1 was 20-30 for just one path.

I'm not sure yet myself, it seems like the event deadlines are more lenient than 1 but I felt strapped for time on day two with minimal running. Try a balance and adjust from there.

Yes. Reloaded a save a few times. Sometimes they throw their hands up and run, other times they keep fighting. But they never die from getting shot in the face. I figure the only point to the pistol is shooting them in the back as they try to run so you can harvest some blood/organs.

If the intent here is to follow a Soulsian “hard is good” philosophy and apply it to the survival genre, this is misplaced. Souls games are about reaction, movement, and practice. You can’t practice finding a piece of bread. If the intent is only to keep the player feeling oppressed, strapped for time, exhausted, hungry and weary, well, that doesn’t mean I won’t also resent having to spend so much time doing the most boring species of meter-management in what could have been a captivating mystery.

Hunger is the worst offender. You can enter abandoned houses and loot them, or even burglarise “healthy” houses. But there’s never any food in the kitchens. There are abandoned houses and free beds everywhere, but you can only sleep in some of them. Put all this together with the samey character models and the clumsy combat, and Pathologic 2 starts to feel less like an interesting failure and more like a budget Skyrim.

The survival mechanics don’t suit anything. The amount of food he ate, and yet was starving by midnight, could be enough to carry you all 12 days. These types of mechanics are fine if they’re realistic, but when you need to consume a feasts worth of food and drink every 12 hours it’s just dumb. And as the writer states, it just takes away from the desire to explore an interesting place and story. Challenge for challenges sake while tossing aside all logic and joy isn’t actually challenge, it’s just tedium and a poor cover up for a lack of truly good gameplay.

Couple of things to try. Check that the install directory is not set to read only. If it is then use the properties menu to disable read only on it. Leave that properties window open as you then try to download the game.

The real issue is the degree of harshness implemented in the game, and the fact there is no difficulty options to accommodate players who aren’t into brutal experiences.

This sort of game seems like it was tailor made for my interests… and then ruined by a poor understanding of what a good and interesting difficulty is.

Unless you’re already starving to the point of being unable to move, there’s no way you’d die of starvation in ~18 hours after that amount of food. That’s really silly! That’s not interestingly difficult, that’s forcing some unnatural fragility because you couldn’t figure out a health meter to mind that actually could change on a short timescale.

Seeing people defend that as “well, it’s what makes the game good because blah blah scarcity blah blah suboptimal choices” is frustrating. It’s not dark and gritty realism. It’s the game equivalent of making sure you feed the parking meter to avoid getting a ticket, except someone has decided your car will rust over and its battery fail instead should the meter go out. That’s not how cars work!

When your hunger bar is full, your health drops fast enough to die within a day. I appreciate the calorie counting, but it’s still punitive and unreal. I don’t really mind a game being unrealistic, but if it is also tedious, that makes the constant face-stuffing harder to forgive.

To address a lot of people’s opinions: I get it. I understand this is meant to be a hard game. But that doesn’t mean it is a good one. And the game would lose none of its oppressive atmosphere if the survival meters, etc, were gone. Because most of the dread comes from everything else. The children in wolf masks, the disease taking over the city, the cryptic and often vaguely hostile dialogue. There are probably more interesting ways to depict hunger than the same meter we’ve seen hundreds of times.

Again, try contacting the devs. They're a friendly bunch and if they can help you get it sorted, they will.

As a long shot, run some disk health checks on the drive also.

It really legit 100% does not matter if you do or don't. The storyline 2, from what I've played so far, deviates pretty heavily from the first game's. The general structure and setting is still the same but almost all character interactions are different.

the simple answer is that bachelor is an outsider like the player, and as such his role is easier to step into. the contrast between going from the bachelor's condescending disbelief in the steppe/Kin traditions and the haruspex's newly rekindled faith in them also makes for an arguably better transition than the other way around.

HD Classic is the original with the tiniest of grafix facelifts and a non-batshit translation. 2 is a ground-up remake of the original. More along the lines of a reimagining.

Anyone know how well the game runs compared to the demo? The demo gave me frequent fps drops that lasted like 10-15 seconds then was fine. Also fps indoors and loading times for indoors was rather bad/long
