Beating the challenge is a big part of the fun for games. If it's not challenging, all you have left is everything else.
Everything else can work and carry a game, but if its the kind of game that lets you choose a difficulty, gameplay is probably a focal point.

Normal. I always play normal because I assume that is what they intended when they designed it. If I like the game I will replay it on the harder difficulty.

Only exception is if I played another game in the franchise, in that case I will bump up the difficulty.

I prefer right, I hate intermediate difficulties because you're neither playing the intended balance nor testing the limits of your abilities. With the right side I can have a normal play through and the hardest possible play through. This is assuming that "very hard" is harder than the left side's "hard". If they're the same just different names then I pick right because its more EXTREME.
