Take your real name, first or last, and rearrange the letters:

>So Dean becomes Dane, Sam becomes Mas, James becomes Jasem.
You'll usually get something weird sounding that will fit into any fantasy setting. If you want throw in an extra letter somewhere:
>Adane, Maks, Jashem

Once you have your fantasy name you can just use it for everything.
