It's a remake/sequel/soft reboot/reimagining. Same setting and story, but supposedly different enough from the original to not be redundant. No one really knows to what extent yet, but we'll find out soon.
Majora's Mask, the time limit is pretty forgiving though, since you can always rewind the three days.

Basically they ran a Kickstarter for a remake of the first game, then the Russian Economy ate shit and it caused a lot of issues including needing to find a publisher in TinyBuild and when they did that it was determined to call the game Pathologic 2 due to the remake having significant differences to the original and wanting to distinct them as two separate titles. Even then the game's still going to be semi-episodic due to development issues caused so the game's only releasing initially with the Haruspex with the Bachelor and Changeling being added later.

It's a sequel remake, the events take place as a second attempt at the play from the first game, canonly you fucked up and the town gets nuked, Pathologic 2 starts at the end of the last day from the original game.
Based on the demos and betas there's a lot of references that it's a second attempt at the same events, think Evangelion OVAs

Marble Nest was a self contained setpiece, it isn't canon in regards to the finished Pathologic 2. Or perhaps it will be by the time the Bachelors route comes out, who fucking knows. But that user was talking about the demo version of the finished build (not the alpha version either, the demo they released a month ago). In that demo, the intro starts on the 12th day with the town decimated by the plague, aka "bad ending", and Haruspex tells Mark he wants to start again from the beggining, to get another shot.

that's similar to the conclusion I arrived to when in the last days of her playthrough your 'sister' talked you about destiny, stealing fate and changing the past and whatnot. I reasoned that she -like aspity- was an embodiement of the plague but when she called herself a miracle-worker and made up having a sister she basically split herself and was now one entity with 2 destinies and the 'other' clara is actually just you fulfilling your other destiny while you fullfill yours. And the fate that gets fulfilled invalidates the other, which is why if you perform the true miracle your 'sister' banishes to the steppe and takes the plague with her something like that. i dunno man this game is an acid trip
