Because the vast majority of games are already entirely barren of mechanics regardless of difficulty settings, no reason to gut the few games that have them when idiots could go play something else.

Having one set difficulty is probably the most effective and underutilized choices in modern game design. I honestly hate deciding whether to play on normal to experience the creators' objective vision of the game or playing on hard for the challenge. Game's with one difficulty usually have both to offer and its one of the many reasons why From's games are so cherished.

there are 2 difficulty modifiers in the game and its your fault for not using them

I'm at Isshin Sword Saint and this fight feels genuinely cheap, he does half my health in one hit, two hits and I'm dead, and one combo takes up nearly the entire posture bar so unless I literally deflect everything I get my posture broken quick as hell. My Vitality level is 17 I believe, and Strength is 12, am I under equipped or what?

No, thats normally how the fight is. You also need to be really far away from him to heal safely, which means his posture will recover. You don't have to deflect everything. Try to bait out the thrusts or avoid parts of the combo. Really, you just have to learn the fight and know how to avoid his attacks and when it is safe to attack.
