It's really not that bad. It's no different from how actual kids would act. I guess that's the "mainstream" part of it. If you're cared for kids before you'd know they take their clothes off just for fun or don't care about being naked. It's usually the adults that panic and scream their heads off about it, otherwise kids don't care. The reason it's weird is that, you have kids in awkward situations being kids in the show targeted to adults. The only moments it's pushed is when the orphans are disciplined in the very beginning (being hung up naked and whipped) but it's extremely brief and it supposed to make the orphanage look like a hell hole. The other is the maid girl (boy) but that's also subtle and brief.
The rest is just general torture and experimentation on kids but it's not sexual torture for the point of masturbation. It's meant for shock value.
