I hate cities in games. The bigger they are, the more I hate them.
I already have to walk on foot IRL all the time to get shit done in my city, why would THAT need to be in vidya? Do you want realistic pissing and shitting as well? Realistic stamina so you can only fight one or two dudes before needing some actual rest?
Best cities in video games are old JRPG cities with 3~5 houses.

Another thing I hate on RPGs are items and bags.
Why do you need them?
Aren't RPGs based on fantasy stories? I've never read a good fiction where characters keep changing gear after every milestone. Just give some gear that keeps upgrading as I level up, each one with its own perks I select as we level.
Potions? No please, never seen a fantasy, movie where characters keep chugging potions in mid battle, so stupid.

This is only bad when you end up with literal tons of trash shit. 18 varieties of "upgrade materials" and tons of in-battle items you'll never use because "what if I need them later"

that's pretty common in LotR, Narnia, Harry Potter, etc
even Greek and Arthurian myths, upgrading gear is a big point in the heroes journey

What's a good size for cities in video games?

Depends on the gameplay and what purpose the city serves.
Does it being big actually enhance it in any way? In a game like GTA or even online games like RO where cities were meeting points, them having size is not bad.
In a game like Diablo where 99% of the gameplay is outside of the city, it being big isn't very useful. I must admit when I was a kid and more impressionable I liked them being big in such games, nowadays I want them as compact as possible.
