why can't we have a 80kmx120km cities with a dense and diverse downtown area and 200x200km suburban sprawl surrounding it, then a 500kmx500km rural area to top it off? And it all unique assets -- except in the suburban areas.

Yeah. I mean most of it was roped off to the main path, but there was a huge city inspired by China's now demolished Kowloon Walled City.

In my opinion, this also applies to games. You can't just spend tons of time developing meaningless areas with nothing in them. If you build a city that size, then you have to fill it with stuff.
We've seen plenty of games not do this and that's why open world games have such bad reputations nowadays. Because they just have lots of meaningless areas.
And some games do have cities that size. Most of the GTA games have cities that size, but those games are usually JUST the city. I assume you're wanting cities + areas outside the city.

Game of Thrones is just anime but with a bunch of sex and bad language thrown in. NPCs will eat anything up as long as its not Japanese
huge cities where you can go inside every building are obnoxious because then you have to go inside every building to see if the devs put something interesting in it. right now I live in a shitty little town with only 2000 residents. I've still only been in about 15 buildings because I have no business in any of the other ones and it would be tresspassing to go inside most of them anyway. in a game though there's a meta expectation that you break into everyone's house just because there might be some collectible under the bed.

I feel like even at a tenth of the size novigrad did a good job of communicating scale in the city, the outskirts are believable and it's great standing far below the top of the city and looking up
What purpose would it serve?
Deus Ex Human Revolution makes you feel you're in a huge city but you can only explore a few neighborhoods and it feels alright because the city is just the backdrop of the story the game wants to tell.
Would having a huge city to explore make the game better? No

Big cities are trash. More filth, more trash, more subhumans. Literally zero reason to live in one irl or in game.
It was way more impressive before flying mounts, when you could only see the city from on the street between the buildings.
WoW has never been lauded for realistic city/town/village portrayal.
Computer performance is limited.
You can only draw so many triangles per second.Then if you add lighting and shadows and reflections and refractions, you have no chance anymore to simulate an area that large in real time.
Instead of 60 frames per second, you end up with many hours per frame.
