RE2's hard mode is absolute bullshit. What's amazing is that the easy mode is actually easy. In easy mode, if you are swarmed by zombies, they can bite you multiple times and you are still labeled as fine condition. Even normal is too brutal at times.
Enemies are given new attacks at higher difficulties.

Stalker like Increased damage for everyone and throw in supplies being decreased as well. I don't have any idea how you would balance a game like DMC without it coming off as a either bs health and damage or a gimmick "everyone dies in one hit" type of thing. I do think that at that point the people who play it probably enjoy those difficulties so who the fuck cares if they are balanced.

Depends on the game. A lot people here say more hp is bad, but games like DMC, Bayonetta, and even a few rpgs benefit from enemies having higher hp because it works well with the combat system. Generally speaking though adding new moves to enemies, mixing up the enemies you encounter, enemy AI getting better, and even changing the level layout are a good start when designing a hard mode.
Depends on the game: for an rpg i'd decrease the skillpoints/exp to force the player to actually specialize in something instead of creating a jack of all trades build. Also increase requirements for skillchecks if present and maybe add unique mechanics like having to eat/drink/sleep to make the player choose if they want to spend money on that kind of stuff or maybe gather resources in the open if possible.
For an action game i'd make unique enemies or patterns for bossfights. All of that would require work though

You need to beat the game to unlock a harder difficulty, then beat it again to unlock the hardest difficulty, and then beat it on the hardest difficulty to get the best ending.

Thief's hard mode is well done in my opinion, it gives you additional objectives to complete and requires you to steal more valuables to complete a level

This + game being so easy it's a boring chore to go through is the sin of jrpgs.
Enemies move faster, more unpredictable and more aggressively. I remember Resident Evil 4 has it like that. The Ganados move faster, are more likely to sidestep and lunge themselves at Leon.

The better question is how would you do easy mode? I feel like too many people tackle this shit in reverse. Balance the game around the hardest difficulty so you know for sure it works, and then just lower HP and omit or change especially harder enemy attacks and enemies outright for easier difficulties. This way, the player is still exposed to some semblance of real challenge thanks to the game being balanced as such, and I'm sure they'll generally find it more fun overall as a result of challenge still hanging around in places.

You legitimately aren't supposed to kill the zombies in hardcore. Shoot them or even flash them with the light to stun them, blow their legs off if you absolutely need to get past them like if Mr. X is chasing you. NEVER fight a Licker, and just pop the Ivy Zombie seed pods so you can run past them.
You pretty much need flashbangs for Mr. X, which is why it pisses me off that the game will auto-equip subweapons without telling you.

game is designed around the hardest difficulty being the standard. remove certain enemy moves and make item placement more generous on easier modes. health adjusted the same. credits encourage players to try harder difficulties like dmc4
The same way Divinity Original Sin 2 did it. Enemies get more defensive and offensive options and have better AI.
