The immortal antagonist, Magusar, is essentially the bro of the book who's written memories you relive, who goes mad due to a demon he absorbed long in the past (which gave him his immortality) he can't really remember into his arm (This is how sorcerers banish demons (humans become demons due to sin/deal with the devil type affair etc.), most of the storyline twists come from it such as a narcissit who turned out to have had to banish his lover, thus his narcisism being the remnant of her feelings in his arm). Throughout the game he's positive that the end of the world can be averted and that there's hope. At the end of the game you absorb Magusar to retun the world to normal, essentially resetting the apocalypse he caused. The last narration is your characterin the fixed world thinking things might be different this time and full of hope, while you as the player obviously know he has the demon in his arm and will eventually go mad himself, causing the cycle to continue.
The true identity of the guy who's memories are written in the book is actually the book itself, deformed after Magusar poured his immortal blood onto him after beating him to near death over and over (Magusar and senpai were best buds trying to avert the apocalypse together before the demon took over, so he didn't want him to die), he become the book so that you, the player, could relive his experiences to train, sacrificing him before the final fight to gain his power for realsies. If you choose to try and save Magusar, those events repeat, and you end up becoming the book senpai for the next prisoner to sacrifice.
