How do you think Undertale came about?

The main plot in general would center around the bandit, though maybe instead contacted by the head of the city to stop a rebellion that has been killing huge numbers of citizens rather than immediately opposing the head of state. The head is an Emma Goldman-grade anarchist, but not Leon Czolgosz crazy, tried embracing the freedom of the frontier to the fullest extent.
In response, let's say that after stopping the rebels, who are maybe seeking a more organized system of government or are just power-hungry assholes, the head of state was assassinated, sort of a Dishonored move; start a problem with the rebellion, make the head look weak through the rebellion, assume control right after. Now our lovable bandit is trying to outrun the new heads while being helped by a ragtag militia from those he saved, and leftover rebels who decided he wasn't that much of a jackass.
Good and bad endings remain, mostly based on how you treat innocent civilians, decide to capture rather than execute enemies, and so on. Absolute good has you leading the survivors out of the mountain to a new life, absolute bad has you ruling over the city as a total tyrant. In-between can be smaller groups leaving, you being exiled, and so on.
For the features, consider the pile of unused ideas and assets Ken threw out of Infinite so he could have more money to pay for Elizabeth SFM. Having actual builds rather than packing everything in one is just the start.
Call it Bioshock 3 as a placeholder until we can find a more original name.
