They make it very clear what happens there, the survivor runs out and tells you he "won the fucking lottery", everybody else is crucified, Vulpes tells you they played the lottery as ironic punishment, Boxcars says he won second place which was being crippled.

The mayor takes some searching though, there are some journals he wrote in the gas station down the road from the Mojave Outpost.

Up to you if you want goofy shit to happen sometimes or not. Only gameplay change directly is finding a YCS Gauss rifle or Limited Ammo Alien Blaster. Both energy weapons.

Even most of the winners merely got slow torturous deaths and he got to watch. All his friends are dead too now. A whole town gone in overnight and only he got to live.
