Nilfgaard. Maybe in the conquered territories like Cintra, or right in the heart of the Empire, just far enough from the Northern Realms so the game could ignore most of player's decisions in the trilogy while also feel more original by exploring new land and culture.

Charachter creation is a must have. They should give the player several witcher schools to choose from which will affect MC's stats and perks and maybe backstory

Actual open world, not multiple locations that are separated by loading screens. More focus on hunting monsters, with actual preparations, like they promised at E3 2014.
