I heard Visceral had a very different vision for the game originally, but EA's meddling ensured it didn't come to fruition. I think whatshisface, Carver wasn't supposed to be in it and Isaac was would've had hallucinations of a "dark" Isaac or something throughout the game. I learned this stuff a long time ago, so it may not be totally accurate.
