let's say attack rolls are done with a d100, any result equal to or lower than your attack skill is a success, while crits are results in a low range, let's say 1-5 or your attack skill/10 and under. fumbles are always in the 96-100 range and some games have players roll in special fumble tables when that happens.

That's not supposed to work that way.

Combat roll is determined by your chance to hit % - enemy's chance to dodge %.
Your chance to hit is combat skill%+agility/5+luck/10 and some other stuff, so it's easy to get it above 100%, and you can buff it with spells, enchantments, etc. so even when fighting enemies with high dodge chance you will still have combat rolls over 100%.
An optional endgame boss has a stupidly high luck attribute, something like 700, which gives him a 70% bonus to his chance to dodge.
