It's based on the Camarilla edition's lite version, which introduces Bloodheal and tweaks Bloodbuff disciplines. You heal at a very slow rate unless you spend a blood to heal yourself and feeding no longer heal you by itself, which makes sense.
Each clan gets their unique short questlines, hence the name of the mod. And the biggest addition is the Sabbat ending path, which is fully voiced and it's written kinda nice. They tried to make the characters sound the same and in all honesty didn't even notice the difference in Andrei's voice. You also get a 5th hub called East LA, which is a sabbat domain. Has an interesting character called Hazel Tomes.

The Clan Quest Mod does a decent job of expanding on the Sabbat and Kuei-jin storylines a bit, which is pretty nice. Right. Get the Unofficial Patch and install the basic version for bug fixes. Don't install the plus version because it heavily mods the game.

Or get the True Patch Gold which is the closest to the vanilla game but harder to set up.

If it's your first time get unofficial patch with its basic installation. If you want to spice things up the Camarilla Edition is pretty good and a lot of mods are based on it.
The Final Nights is fun and it introduces various clans and smaller bloodlines. It changes pretty much all the music in the game and it might be a turn-down for some people but having played this game probably over twenty times by now I found it refreshing.
Clan Quest does the most changes, introduces an additional Sabbat questline to the main-quest.

I've been playing without mods up to chinatown and no crashes or gamebreaking shit so far. It is damn janky though. Especially collision with, with some objects getting you completely stuck and some npcs blocking pathways. Dunno if any patches fix that.
