The guy in the video clearly used the mod in the description to make his own custom hud for the Bloodborne mod he is making, there isn't going to be a standalone version of it unless he uploads it.
He used Custom UI Replacer and most likely the UI was made by the uploader of the video, there is a dark souls one included though.
No proper up-to-date guides currently exist. If they did, I wouldn't fucking be asking. People here tend to have the most obscure files for the most obscure games of all time. I'm also not kidding about buying you cucks a game. There is a link out there. It's been shared among koreans that STILL play this game while streaming w/ the same preset.

I think from the comments that the whole thing seems to be a mod he's working on, if so it's just a waiting game for if he releases his work or not. He's shown the mod he used to replace his hud bit he hasn't dropped any files concerning the actual hud itself, only the tools, I think he's the only one who has it user you're just gonna have to wait for a response or for him to possibly release, sorry man.
