I accidentally killed a good orc in Shadow Of War once.

the original utawarerumono had a bad one, luckily I'd already watched the animu and knew not to level him

In TBC while leaving Thunderbluff on a flightpath, I happened to witness the local chat for Bloodhoof Village while it was under attack. There was a lvl 6 tauren standing there pleading his heart out near the npcs the alliance was killing, "Please help they're killing my friends.", "Stop killing my friends.", "You're hurting them, please stop."

I felt that really hard for some reason.

I really like the game overall but those points along with the god awful crafting system really made no sense

The choice sort of made me think on what price victory. Assuming I wanted my homeland to win a war and make a better life for us, how far should I go to see it happen?

I went chaos in the end, completely uncompromising on my ideals, but I can't help but feel like I doomed everyone for doing the right thing. The only thing that felt like bullshit was Vyse's part to play, since he betrays you no matter what you do. At least with Order, he felt incredibly sympathetic because he was trying to do the right thing, and although you became a monster, you were still trying to focus on the bigger picture. In chaos, he just becomes a monster for no reason since his actions ended in failure.

Alessa dying at the end of SH1 after you learn everything she went through. And Mary from SH2 if you count the letter being read at the end.

The death itself wasn't so bad, but he asks you to pretend to be him since he was a high ranking general and the country is at the brink of civil war. You even meet his mother a few times and right before you head into the decisive battle she tells you that she knew all along that he was already dead when you first met her and then she asks you to tell her about his last moments.

God, that scene was one of the most satisfying things I've ever seen in a game.
