Fortuna is a very small town, it's not like the entire island was populated. so much so that not knowing who someone's father or mother is is unusual as everyone knows who everyone is, there's probably only a few thousand people at most in the entire town. Plus who knows how many people actually died, DMC4's incident only lasted a day while the Qliphoth incident (DMC5) lasted a little over a month. Also in the novel it says that Trish helped protect most of the civilians in a magic barrier or some shit, and the knights of the Order also prpbably helped evacuate most of them.
