Not really, yeah there's the alternate ending, but once you finally get good at timing counters, it all just becomes easy even with the bell demon and no kuros charm.

On my NG+4, for the Return ending and final few achievements I did today, I did him in one go. What a fun game and a fun fight to top it all off with. I'm no hyper autist that makes him trivial, so it was a fun fight, even tho' that was the third time I fought him.

"New Game+" modes are an extremely rare mechanic, and that coupled with no Kuros Charm and Demon Bell give this game insane amount of replay value.
Also NG+ modes actually have new enemies in Sekiro, just like DS2 did (the one thing DS2 did well, NG+).
