Hire Avellone, Sawyer and some other key people from New Vegas's team over to Bethesda and put them in charge of narrative and writing, and have them oversee quest and world design, while letting the Bethesda people handle core gameplay and heavy lifting of engine and asset creation

Use a MacGuffin like New Vegas so you can create whatever backstory for your character, and that there is an over arching plot.
Revamped version of Fallout 2 S.P.E.C.I.A.L. to have traits come back.
Combat from Fallout 4 with some heavy modifications, the base for it is pretty ok but needs some fixing.
Hire some fucking from netflix shows/basic cable TV. Combine with industry veterans. Decent writing goes a long fucking way to making a decent video game.
