It's more than that. A major part of the gameplay is that the characters rapidly cycle between four ages, child adult middle and elder. Any scene where a child got hurt or a character acted like an adult while in child mode (like pigtail girl's obsession with double entendres) were cut.

well I read spoilers on the japanese release and I dont know much about the characters so all I can do is give you a gist.

The doctor and the little girl are brother and sisters from different mothers. Doctor got swole so she doesnt recognise him. Also she's a clone of his real little sister with none of her previous memories. Their father was the one who self destructed the world using nanomachines to turn people into monsters because he wanted to sell his technology for war by turning corpses into moving monsters and stuff but people wanted to stop him. At the end he's the one who built the cloning machines and everyone keeps aging fast because they're using the old keys that are built for disposable clones that age quickly for security sake. In the end he appears as the last boss, merges himself into a perfect clone body. You beat him and he breaks the little girl's X key as a last resort because she's the only one who uses a later version of the key that allows her to age normally and to have kids. Everyone uses the last cloning machine part to revive her as a baby and raise her up again as their parents. The cloning machine stops working and they all died off after teaching her everything they knew. She left the island to look for survivors because the doctor lied to them about them being the only humans left.
