Nia was the blade of either her 'sister' or 'dad'. She wasn't actually related to them, but was pretending to be a part of the family for some reason that isn't really clear.
The dad had her consume part of her 'sister' and become a flesh eater, so she could live on through Nia.

We don't see any of the events leading up to Nia becoming a flesh eater, only the aftermath. Nobody knows if she became a flesh eater before the girl died or after.

Yeah, it was about how Haze looked a lot like Lora. Also explains why Finch looks like a bird (her blade quest hints she used to be bonded to a Tirkin) and Wulfric looks like a bug (used to be bonded to an Arachno). It's also possible Dahlia used to be a snow Bunnit's Blade and Nim most likely was bond to a Phenex at some point early on, but neither of their quest delve into their past Drivers so there's no hard evidence.

It's brought up a couple of times. Addam comments on how uncommon it is for a Driver's appearance to be so strongly reflected in their blade in the flashback at the start of chapter 5 (in reference to Lora and Haze), and after Rex says talking to Amalthus felt like talking to Malos, Gramps tells him that some Blades' appearance and personality are similar to their Driver.

I’ve learned not to overthink things. Like how Rex can open Leftherian doors despite not actually being Leftherian. Or if Rex is Leftherian, then who are all the brown people in Fonsett Village?
