It has extremely low texture quality that has to be hidden through dim lighting effects, which is probably why it even runs on toasters. Why does everything in every single Unreal Engine since I think 2 look like it's made out of plastic? There is something HORRIFYINGLY unnatural about it. It can fit (or at least does not hinder) highly cartoony games and generic sci-fi stuff, but it everything else looks fucking attrocious.
And it's a problem that seems to be pervasive with the Unreal engine Games through out different iterations, and it's way too universal for it to be just an issue of incompetent art direction.

Its highly competitive with all the best inhouse engines and likely more versatile than all of them. You'd have to be a retard if you don't think Unreal Engine is good. Best of the best is debatable, but its objectively among the best. Not to mention all of the support that Epic provides for it including sending people to dev studios just to teach the devs the engine and help them get started.

A major aspect of visual fidelity, amd realism, or style, is all owed to the middleware used to produce those assets prior to import, granted within the tech constraints of the engine.
But I'd say it's mostly down to the skill of the artist and tools being used -- 3DS Max, Maya, Blender, Zbrush, Mudbox, Photoshop, world engine, and etc... Not to mention the vast ecosystem of plugins
It's accessible and fairly cheap. That's why it's often used. But for any developer serious about their game, they should strive towards making a unique engine for the game.
Tekken 7 for instance looked soulless as fuck due to it being on UE4. It needed a new engine.

Ace Combat 7 is UE4. Funny how an arcade flight game manages to stomp dedicated flight sims in making realistic looking clouds.
Tekken 7, Dragon Quest XI, Soul Calibur 6 and Ace Combat 7 are all on Unreal Engine 4 and they're gorgeous looking games. They also run smoothly.
Kingdom Hearts 3 is also on UE4 and it looks really nice most of the time.
Even Final Fantasy VII remake will use it and that game already looks beautiful.
All things considered it's a solid engine.
