Has a lot to do with the lack of looking into moderator candidates. At some point what ever team that was interested in running the forum has become incredible burned out and hardly interact with the community at large. However the forum is still active so they need a new moderator offload work onto and that new moderator might not be vetted as well. There is also a bit of nepotism involved on every level no matter how hard you vet. The most active people and members who are friends with staff get the most exposure and thus the most informed opinion on potential recruitment.

Also the site owners involvement heavily dictates the success of the forum, site owners come in many varieties. The absent father is most common, leaving unpaid janitors basically in control. If the next highest ranking unpaid active staff member of this isn't of decent ilk, this causes a massive downward spiral. Site owners could still be involved but massively incompetent, they speak another language and have poor language skills in general, communication with them would be easier in binary, they fail to understand that you can't just put garbled english as the new site domain and think it would be better for the search rating. These are the kinds of retards that won't get google analytics so we can measure and monitor our growth, but are worried about the word incest lowering the sites google ranking.

Even among good staff, burnout, arguments that leave everyone bitter will spiral out of control at a point and require a massive overhaul and strict evaluation of behavior. However this only happens if the staff recognizes the issue and fixes it, but losing perspective is something that goes away when you are involved in community moderation for long enough, no matter how diligent. You can't just fire everyone and get new people, your community isn't large enough and you can't vet 30 new people at a fast enough rate that it would be worth it.

Some mods do their job well and enforce the rules, others ban because of their butthurt or power trip.

Sometimes a mod will cleanse the thread of /pol/tardation and totally-not-porn dumps. Other times there will be a /pol/tard thread and a non-vidya bait thread on page 0, but a vidya thread will get deleted because someone said something mod disliked in 100th post in said thread.
