deflect the sword, he'll fall over for 5-6 seconds if you deflect his overhead swing

Same as stage 1, charge around and sneak hits on his backside whenever possible.
Use charged spear R2 for a special attack.
His scream has a big wind-up
Watch out for big sweep attacks.
Once he's below 50% vitality, start focusing on deflecting attacks for posture damage.

Like for literally every other boss, be on his ass constantly, when you're not parrying, attack him. When you see he's going to do the screech, sprint away.

The animations on stage 2 are really cool, like he's a marionette being controlled by the centipede but the attacks themselves are retarded. He doesn't even swing the sword he holds half the time, just awkwardly wiggles it around in front of him, yet somehow you still get slashed by it.

you can do it the easy/cheesy way
just run away all the time until he does the dive attack, that's when you jump towards him and hit him a couple of times and run away again, you can toss some shurikens while running away as well

Hell no nigga, keep attacking, 3-4 attacks will stagger him and give you free damage, by the time he screeches again your terror bar will go down. If not then use the terror gourd. Doing this trivializes the 2nd phase.

After you perfect parry his telegraphed overhead smash and he falls over stunned, use the spear prosthetic to rip out the centipede for big posture damage.

Man the headless ape cleaned the floor with my face, it was the boss that i had most difficulty till now.
