You never died on any of my playthroughs, I don't know what the problem is.

I don't really see what the point of the final mission and party members dying was really, wasn't it just a case of if you didn't do their side quest they would die? Or if you picked the wrong person for the team? Why would you not do everyone's side quest?

theres like 3 audio logs at the end of the game, two of them with liara talking and one that plays reaper sounds
