It's a hidden quest. You need to choose a number of specific traits, fail checks with a few special NPCs and, of course, need to have created your character in a gun-heavy starting area.
It's really difficult to get. Some people call it game over cutscene because you lose control of your character.

To be fair, most of the cool shit is somewhere in that 70% and most of it is untouched because of the inane grind it takes to get the equipment to be able to get to it, pretty much have to dedicate your entire skill tree to it. At least other players who did are willing to screenshot everything they do while exploring those parts of the map.

Bro everybody in this game is a narcissist. It's just the nature of the game. The trick to building your CHA stat quickly is to make others think you're not a narcissist. The better you are at disguising it the higher the stat will grow. Doing sidequests that give virtue points for charity guilds is a common way of doing it that a lot of players use.
