Fix the algorithm for matchmaking, better balance heroes, and fucking get it drilled into people's heads they need to switch more often.
The last one bugs me the most. Game is 3 years old, yet people pick shit like triple snipers into a rein+orisa, or see the enemy has three healers and don't reach for the ana.
particularly it just ends up amounting to 80% of games being complete stomps. Even when you're on the winning end of that it's pretty boring, and it just seems like carrying is also ridiculously hard now compared to before. 10k+ dmg or healing just may not be enough especially if your team just does fucking nothing.
As you can tell I just love the game.

Tone down all ults, or at least make them take a lot longer to build up and greatly vary it. None of this bullshit where everyone passively builds up their ult so they all get it at the same time. Make people have to think if they want to burn a weaker ult twice, instead of saving it for a team combo. Maybe youd get more 3-ult combos instead of everyone waiting for the 6.
Allow VOIP without moderation. Give people the ability to mute others and thats it. Completely prevent anyone from reporting others for anything other than video proof of throwing a match. Restore dance/taunts whatever. The game used to be fun when you could talk shit etc. Forcing everyone to 'play nice' doesn't work when everyone is so fucking hell bent on winning, that they will scream at you for picking a hero you prefer. So fuck em, mute them but the other player can't be reported either.
There's a distinct reason why the game dropped like 95% of its player base within a year. Turns out banning fun while encouraging the idea that people picking their fav hero is something banworthy, kills a game.

The biggest issue is more than after sombra and ana, the new charaxters don't really feel like they were designed with the original cast in mind.
Granted, they're all fun to play but they really don't help the game much. Brigg and un-nerfed fist in particular.
Prior to that even fi you were up against a counter you weren't innately screwed, you could outplay them. Now though you got shit like brig stunning any diver instantly, reaper now just winns almost every 1v1 outside of mcree getting the jump on him. It's just become a frustrating environment. particularly I don't think we needed more CC. Mei, Mcree, Ana, and Sombra all brought enough and in different ways.

buff nonmobile dps/tank hp across the board, leave the mobile characters as-is
fix movement: remove adadad, correct airstrafing, give propulsion and thrust proper momentum carry and reduce air drag
add crit heals: buff hps across the board but greatly reduce the amount of healing received while in combat so that any healer can quickly top off tanks but only in between fights
